Nurse's Office Update

June 2019


Incoming 7th and 8th grade Physicals 


Junior High Physicals will be held on Monday June 10th and Monday June 17th 2019  from 6-8pm in the Springs School Nurse's Office, at no cost to the student.

These physicals are for students:

   -entering the 7th grade and will fulfill the 7th grade physical requirement, regardless   of whether they plan on playing a sport or not.

   -entering the 8th grade in September 2019 who would like to play a sport in the         2019-20 school year.

A parent or guardian must accompany each student to the physical.


Student Medications

Any students with medication kept in the nurse's office must have a parent come into the Nurse's office on the last day of school to pick up medication. No medication will be kept in the office over the summer. All medication will be disposed of on the last day of school.

If your child will need medication for the 2019-20 school year, new orders are required and medication will need to be dropped off at the start of the new school year. Please check the website for the necessary medication form that needs to be completed by your child's physician.



Please review physical exam grades below and

schedule physical examinations accordingly. 


Health examinations will be required for new entrants and in grades PreK or K, 1, 3, 5 and 7.



Vision screening for color perception, distance, and near vision acuity will be required for new entrants within 6 months of admission to school.

Distance and near vision acuity will be required in grades Pre K or K, 1, 3, 5, 7, and 11.

Hearing screening utilizing pure tone testing will be required for new entrants within 6 months of admission to school and in grades Pre K or K, 1, 3, 5, 7, and 11.

Scoliosis screening will be required in grades 5 and 7 for girls and grade 9 for boys.

Changes for screening grades will be implemented for the 2018-2019 school year. 




New York State Public Health Law now requires ALL STUDENTS ENTERING 7TH GRADE IN SEPTEMBER to receive a Meningococcal Vaccination. Please make your appointments early rather than later. Please bring proof of immunization to the nurse's office.

Please schedule an appointment for the immunizations needed as soon as possible.  

Once your child gets the necessary immunizations please forward the paperwork to the nurse’s office.  It is extremely important that this documentation is in your child’s file.
