Physical Examination Requirements

New York State Law requires that all new entrants (whether Pre-K or K), 1st, 3rd, 5th and 7th graders and all students entering a school district for the first time have a health examination must be provided to school within 30 days of entry into school or the grade levels indicated above.

Acceptable health certificates may be dated anytime within the 12 months prior to the start of the previous school year. If a copy of the health examination is not on file within 30 days, you will be notified by the school nurse.

We recommended that this examination be made by your family health care provider who knows your child best. If your child has a physical exam scheduled during the school year, please notify the Health Office with the date of the scheduled exam.

For Junior High Parents

If your child is entering the seventh grade in the fall, a physical exam is required per regulations of the New York State Department of Education regardless of sports participation.

Eighth graders who plan to play a sport also need a physical exam. Physical exams can be completed by the school physician or by the child's family doctor.

Sports physicals will be provided by the school, free of charge, in June. Please check the school website in June for these dates.

A sports physical form along with additional paper work is required for sports participation and can be obtained from this website on the left-hand side menu, under the Sports Physical Packet link or at the Nurse's Office.
