SCREENINGS:
Hearing, vision and scoliosis screening is required by the state of New York to be done for students in pre-K through 8th grades and all new students to the district within 6 months of admission to school.
Distance and near vision acuity are required in grades Pre K or K, 1, 3, 5, 7, and 11.
Hearing screening utilizing pure tone testing is required for new entrants within 6 months of admission to school and in grades Pre K or K, 1, 3, 5, 7, and 11.
Scoliosis screening is required in grades 5 and 7 for girls and grade 9 for boys.
If your child fails either the vision, hearing or scoliosis screening completed at school, paperwork will be sent home. Please take your child to the doctor to see if there is a problem and return the paperwork to the school nurse. For hearing problems, a visit to the child’s regular doctor would be the first step; that doctor may refer your child to a specialist. Vision problems will require a visit to an ophthalmologist or optometrist. If help is needed, please contact your school nurse.

New Required Physical Forms
Beginning on 1/31/21, schools cannot accept the health exam if it is not on the required form or the required health record equivalent.

Students who are entering 6th grade in September must receive the following immunization:

Please schedule an appointment for this immunization as soon as possible. September is not that far off and doctor’s offices will be very busy immunizing students for the start of the next school year to meet these new state mandated requirements.
Once your child gets these immunizations please forward the paperwork to the nurse’s office. It is extremely important that I have this documentation in your child’s file.

7th grade is also a mandated physical year. Please schedule your appointment early and make sure to bring the proper sports physical paper work if your child is interested in playing a sport in 7th grade. Forms are available on our website and in the nurse’s office. Sports physicals will be provided by the school, free of charge, in June. You will be informed of the specific dates, as soon as they are available.

Por favor, programe una cita para esta vacuna tan pronto como sea posible. El mes de Septiembre no esta lejano y las oficinas del doctor estarán muy ocupadas vacunando a los estudiantes para el comienzo del próximo año escolar.
Una vez que su hijo reciba esta vacuna, por favor envíe el papeleo o pida que la oficina del doctor lo envíe por medio de fax a la oficina de la enfermera. Es muy importante que tenga esta documentación en el archivo de su hijo.
7º grado es también un año de examen físico obligatorio. Por favor, programe su cita temprano y asegúrese de traer gratuitmente en Junio los documentos apropiados para que su hijo participle en los deportes durante el 7º grado. Los formularios están disponibles en nuestro sitio web y en la oficina de la enfermera. Los exámenes físicos deportivos serán proporcionados por la escuela, gratis, en junio. Student Medications
Any students with medication kept in the nurse's office must have a parent come into the Nurse's office on the last day of school to pick up medication. No medication will be kept in the office over the summer. All medication will be disposed of on the last day of school.
If your child will need medication for the 2021-22 school year, new orders are required and medication will need to be dropped off at the start of the new school year. Please check the website for the necessary medication form that needs to be completed by your child's physician.
Please review physical exam grades below and
schedule physical examinations accordingly.
Physicals:
Health examinations will be required for new entrants and in grades PreK or K, 1, 3, 5 and 7.
Screenings:
Vision screening for color perception, distance, and near vision acuity will be required for new entrants within 6 months of admission to school.
