  The nurse's office will begin state-mandated vision and hearing screenings of grades K, 1, 3, 5 and 7 on Monday September 23th. 

 

La oficina de enfermería comenzará los exámenes de visión y audición ordenados por el Estado en los grados K, 1ro, 3ro, 5to y 7mo el Lunes 23 de Septiembre.

 
Vision

  • Distance and near acuity for all newly entering students and students in Kindergarten, Grades 1, 3, 5 and 7.
  • Color perception screening for all newly entering students.

 Hearing

  • Hearing screening for all newly entering students and students in Kindergarten, Grades 1, 3, 5 and 7.

 Scoliosis 

  • Scoliosis (spinal curvature) screening for all girls in grades 5 and 7.

 A letter will be sent home if there are any findings on the screening done at school that would cause concern or need medical follow-up.  Please call the school’s Health Office if you have any questions or concerns.

  • Distancia y agudeza cercana para todos los estudiantes recién ingresados y los estudiantes en Kindergarten y grados 1,3,5 y 7.
  • Percepción de color para todos los estudiantes que recién ingresan.

 Audición

  • Examen auditivo para todos los estudiantes recién ingresados y estudiantes en    Kindergarten y grados 1,3,5 y 7.

 Escoliosis

  • Examen de escoliosis (curvatura espinal) para todas las niñas en los grados 5 y 7.

Se enviará una carta a casa si hay algún hallazgo en el examen realizado en la escuela que pueda causar preocupación o necesite seguimiento médico. Por favor llame a la Oficina de Salud de la escuela si tiene alguna pregunta o inquietud.

 

Student Medications

If your child will need medication for the 2019-20 school year, new orders are required and medication will need to be dropped off at the start of the new school year. Please check the website for the necessary medication form that needs to be completed by your child's physician.

 

Please review physical exam grades below and

schedule physical examinations accordingly. 


Physicals:

Health examinations will be required for new entrants and in grades PreK or K, 1, 3, 5 and 7.

 

Screenings: 

Vision screening for color perception, distance, and near vision acuity will be required for new entrants within 6 months of admission to school.

Distance and near vision acuity will be required in grades Pre K or K, 1, 3, 5, 7, and 11.

Hearing screening utilizing pure tone testing will be required for new entrants within 6 months of admission to school and in grades Pre K or K, 1, 3, 5, 7, and 11.

Scoliosis screening will be required in grades 5 and 7 for girls and grade 9 for boys.

Changes for screening grades will be implemented for the 2018-2019 school year. 

  

 

NEW IMMUNIZATION REQUIREMENTS FOR STUDENTS ENTERING 7TH GRADE SEPTEMBER

New York State Public Health Law now requires ALL STUDENTS ENTERING 7TH GRADE IN SEPTEMBER to receive a Meningococcal Vaccination. Please make your appointments early rather than later. Please bring proof of immunization to the nurse's office.

Please schedule an appointment for the immunizations needed as soon as possible.  

Once your child gets the necessary immunizations please forward the paperwork to the nurse’s office.  It is extremely important that this documentation is in your child’s file.
